A medical examination (preventive medical examination) is a mandatory type of medical examination, which is established for enterprises conducting a certain type of activity in accordance with Order No. 302n of the Ministry of Health and Social Development dated April 12, 2011. It makes it possible to predict the development of an occupational disease and helps organizational leaders develop recommendations on strengthening measures to protect personnel from occupational diseases. Order No. 302n with all its annexes today is the regulatory framework for conducting medical examinations. Also, the mandatory implementation of medical examinations is enshrined in Art. 24 of the Federal Law “On the Fundamentals of Protecting the Health of Citizens in the Russian Federation”, as well as Art. 69, 213 of the Labor Code of the Russian Federation.

Types of medical examinations
Preliminary examination (primary) Conducted upon hiring to determine a person’s suitability for work in a particular field of activity. Allows you to identify the subordinate’s resistance to the specifics of the organization.
Periodic medical examinations are carried out by the employer in order to monitor the health status of subordinates over time, timely detection of diseases, initial forms of occupational diseases, early signs of the impact of harmful and (or) hazardous production factors on the health of employees, and the formation of risk groups for the development of occupational diseases.
Extraordinary For this type of medical examination, the frequency is not established by any regulatory legal acts. It can be carried out at the initiative of employees (based on their complaints about deteriorating health), at the initiative of employers if there is a suspicion of deterioration in the health of subordinates. And also according to the recommendations of the medical organization for individual employees who, after undergoing a medical examination, were diagnosed with a borderline health condition.

Various types of studies may be part of a mandatory medical examination. The list of such studies may differ depending on the type of activity and harmful factors that a particular worker deals with at work. As a rule, the following types of studies are prescribed: fluorography, cardiogram, general clinical urine and blood tests, etc.
The employer can also initiate the inclusion of additional tests and examinations from doctors.
The order of the Ministry of Health and Social Development of the Russian Federation No. 302n dated December 5, 2014 regulates the procedure for conducting mandatory, preliminary and periodic medical examinations or examinations. Harmful and dangerous production factors and work that require such examinations are listed. The order contains information about what medical contraindications there are for admission to certain types of work.
Which employees are required to undergo a medical examination?
At the initiative of the enterprise management, people of any profession can undergo medical examinations. According to the law, it is mandatory for workers in the following professions to undergo regular medical examinations:
- employees performing steeplejack and high-altitude work;
- working in the food industry, public catering;
- workers of children's, educational, medical, preventive institutions;
- employees of sanitary and hygienic institutions (which include beauty salons, saunas, laundries, etc.);
- employed in the production of pharmaceutical companies;
- cleaners;
- persons whose activities are related to water treatment and water supply systems;
- workers of poultry farms, livestock enterprises;
- employed in hazardous or hazardous work; persons under the age of 21.
What documents are required to undergo a medical examination?
To undergo any type of medical examination, you must have the following documents with you:
- Passport or ID
- Direction for passage from the organization
- Outpatient card
- Extract with the results of previous examinations
- Conclusion of a psychiatric commission (on request).
Responsibility is provided for the lack of a professional examination among employees of enterprises and organizations.
The Rospotrebnadzor inspection regularly checks all enterprises for violations of labor laws. An organization can be fined up to 50 thousand rubles and even have its activities suspended for up to 90 days if employees do not undergo a mandatory periodic medical examination. The head of the company himself will also be subject to a fine of 1 to 5 thousand rubles and may be suspended from office for a period of 1 to 3 years.
Stages of conducting a medical examination at an enterprise
Medical examinations at enterprises are carried out in stages. Their organization is based on special medical regulations.

Stage No. 1. Preparation
Determination of contingents Employees of the organization create a list of all professions of the enterprise indicating harmful and/or hazardous production factors and work in accordance with Appendix No. 1, Appendix No. 2 of Order No. 302n of the Ministry of Health and Social Development of April 12, 2011.
The list of contingents is determined on the basis of a special assessment of working conditions. The list includes only workplaces classified as hazardous and/or dangerous.
The list of contingents indicates: the name of the employee’s profession; name of the harmful production factor (Appendix No. 1 of Order No. 302n), list of works (Appendix No. 2 of Order No. 302n).
The compiled list of employees subject to preliminary and periodic occupational examinations must be sent to the Rospotrebnadzor Office within 10 days.
This stage is usually carried out by the Customer. If compiling a list causes you difficulties, then our managers are always ready to assist you in determining the contingents.
Compiling a name list of your employees Based on the list of contingents, a name list of persons who require a preliminary or periodic medical examination is compiled, which indicates: last name, first name, patronymic, profession (position), name of a harmful production factor or type of work, name of a structural unit (if any ).
No later than 2 months before the agreed date, provide our company with a compiled name list of your employees. We will agree and formalize the start date of the medical examination, develop a calendar plan for examinations, and form a permanent medical commission. We conclude an agreement between our organizations to conduct preventive medical examinations.
Familiarization of all categories of subordinates with the calendar plan, issuing referrals to persons undergoing a medical examination. A referral for a medical examination is drawn up by the employer and issued to employees against signature.
Stage No. 2. Carrying out a medical examination
Regardless of the specifics of the organization’s activities, the following diagnostic procedures are carried out for all subjects:
- Clinical analysis of urine, blood;
- Examinations of workers by an occupational pathologist, therapist, narcologist, psychiatrist, gynecologist (for women);
- Digital fluorography or radiography in two projections. The images are taken at the clinic, or using a mobile fluorograph at the Customer’s premises.
- Electrocardiography;
- Mammography or ultrasound of the mammary glands (once every 2 years for women over 40 years old);
- Bacteriological, cytological studies (in women);
- Biochemical screening.
According to Order No. 302n, the list of required studies depends on the harmfulness and/or danger of the organization’s production factors. Managers of Altamed+ Group of Companies will compile for you a detailed list of studies required for employees of your company to undergo.
Stage No. 3. Summing up and drawing up the final act
As a result of the survey, for each surveyed employee the following is compiled:
- Patient's medical record according to form No. 025/у
- Employee health passport
- Conclusion based on the results of a preliminary or periodic medical examination
- Personal medical records. They are issued in cases in accordance with the order of Rospotrebnadzor No. 402 dated May 20, 2005.
No later than 30 days after completion of the inspection, we summarize the results and draw up a final report (four copies), which is sent to the territorial department of Rospotrebnadzor. After approval of the act, the professional examination is considered completed.
Medical checkup
Preventive medical examination includes:
- survey;
- calculation based on anthropometry of body mass index;
- measurement of blood pressure in peripheral arteries;
- study of the level of total cholesterol in the blood;
- determination of fasting blood glucose levels;
- determination of relative cardiovascular risk in citizens aged 18 to 39 years;
- determination of absolute cardiovascular risk in citizens aged 40 to 64 years;
- fluorography of the lungs or x-ray of the lungs once every 2 years;
- electrocardiography at rest during the first preventive medical examination, then at the age of 35 years and older once a year;
- measurement of intraocular pressure during the first preventive medical examination, then at the age of 40 years and older once a year;
- examination by a paramedic (midwife) or obstetrician-gynecologist of women aged 18 to 39 years once a year;
- appointment (examination) based on the results of a preventive medical examination, including an examination to identify visual and other localizations of cancer, by a doctor.
Medical examination includes:
for citizens aged 18 to 39 years - once every 3 years:
- carrying out a preventive medical examination to the extent specified in paragraphs 1-11;
- carrying out screening activities aimed at early detection of cancer;
- conducting a brief individual preventive consultation with a general practitioner;
- appointment (examination) by a general practitioner based on the results of the first stage of medical examination, including an examination to identify visual and other localizations of oncological diseases, including examination of the skin, mucous lips and oral cavity, palpation of the thyroid gland, lymph nodes, in order to establish a diagnosis, determine health groups, clinical observation groups, determination of medical indications for examinations (consultations) and examinations as part of the second stage of medical examination;
for citizens aged 40 and older - once a year:
- carrying out a preventive medical examination to the extent specified in paragraphs 1-10;
- carrying out screening activities aimed at early detection of cancer;
- general blood test (hemoglobin, leukocytes, ESR);
- conducting brief individual preventive counseling;
- appointment (examination) by a general practitioner based on the results of the first stage of medical examination, including an examination to identify visual and other localizations of oncological diseases, including examination of the skin, mucous lips and oral cavity, palpation of the thyroid gland, lymph nodes, in order to establish a diagnosis, determine health groups, clinical observation groups, determination of medical indications for examinations (consultations) and examinations as part of the second stage of medical examination;
List of screening activities aimed at early detection of cancer:
- screening for the detection of malignant neoplasms of the cervix: ● examination by a paramedic (midwife) or obstetrician-gynecologist once a year; ● at the age of 18 to 64 years inclusive - taking a smear from the cervix, cytological examination of a smear from the cervix once every 3 years;
- screening for the detection of malignant neoplasms of the mammary glands: at the age of 40 to 75 years inclusive - mammography of both mammary glands in two projections once every 2 years;
- screening for the detection of malignant neoplasms of the prostate gland: at the ages of 45, 50, 55, 60 and 64 years - determination of prostate-specific antigen in the blood;
- screening for the detection of malignant neoplasms of the large intestine and rectum: ● at the age of 40 to 64 years inclusive - examination of feces for occult blood using an immunochemical qualitative or quantitative method once every 2 years; ● at the age of 65 to 75 years inclusive - examination of stool for occult blood using an immunochemical qualitative or quantitative method once a year;
- examination to identify visual and other localizations of cancer, including examination of the skin, mucous lips and oral cavity, palpation of the thyroid gland, lymph nodes;
- screening for the detection of malignant neoplasms of the esophagus, stomach and duodenum: at the age of 45 years - esophagogastroduodenoscopy.
